Stuck in Creator Enrollment - W9 Certification Page

I’m unable to complete my creator enrollment because the W9 certification page seems bugged.

I’ve filled out all information on the page, but still receive errors telling me the fields are blank.

When I click submit, there’s also a javascript error in my browser console:

Have tried multiple browsers, starting the process over, etc. Can’t get past this page. Any advice?
