Summary
I had given up and thought this was my issue, but it’s in Smugglers Gambit bigger than life.
This is for the Star Wars weapon/item bar.
When a player dies, most of their items show on cooldown when they respawn, and they stay that way until the item is dropped and picked back up. See screenshot.

Notice that your weapons are now permanently showing as on cooldown. They still work, but it is confusing to players.
Expected Result
Weapons and items should display normally
Observed Result
They have the Cooldown overlay on top of them
