Summary
When Island Settings → CustomInventoryConfiguration is set, consumable and utility items are destroyed on respawn and the inventory slot they were in becomes permanently unusable for the rest of the match.

Put a Shield Potion and a weapon in the map
Start a session, pick up both, then eliminate yourself
The weapon is still there. The Shield Potion is gone, and its slot cannot be used again.
Things I already ruled out: the respawn method (I tested using Fortnite’s own respawn instead of my Verse one), item removers and other devices (I disconnected them all), and Infinite Consumables. The bug follows that one setting and nothing else.
Tested on ++Fortnite+Release-41.20, Windows 11.
Expected Result
With Drop All Items On Death = Force Keep, the player should keep those items through the respawn. And in any case, a slot that is empty should always be usable again.
Observed Result
After respawning, items like Shield Potion, Chug Splash or Impulse Grenade are gone from the inventory.
The slot they were in looks empty — there is no weapon or item inside it — but nothing can be put in it any more. Picking something up off the ground, or receiving an item from a device, simply will not go into that slot. It stays unusable for the rest of the match, so after a few deaths a player can end up with an inventory where most slots are dead.
Weapons are never affected. It only happens to consumables and utility items.

Additional Notes
The property causing it is Island Settings → CustomInventoryConfiguration, which I had set to Epic’s ItemizationConfiguration_BRStyle. By default this property is empty.
If I clear the property and restart the session, everything works normally. If I set it again, the bug comes straight back. I tested this in both directions several times.
