Summary
First the issue started with rank 6 challenge locations where the it would make a ‘gong’ sound upon completion but not show the text ‘COMPLETE’ or give a chest, it would also only spawn this rank challenge with ‘Hit the targets’ and no other objectives will spawn with the challenge. It has now progressed to all rank challenges and chaos portals/challenges, and dummy challenges.

Attempt a challenge anywhere.
Expected Result
Receive rewards and then after awhile the challenge will regenerate with a new type of objective.
Observed Result
Does not give rewards, does not say ‘COMPLETE’, does not respawn without relogging into the match, most non chaos challenges only have ‘Hit the targets’ objective.
