AI Navmesh failing to generate in 4.21.1

It seems in editor after a few runs the Navmesh fails to generate at runtime near the AI that need to use it to navigate.

In my cases happens in editor randomly after a few runs, run when the map loads in and in a packaged game usually all the time.
Here is what it looks like broken.
