I downloaded UnrealEngine-1.9.1-2352264.dmg for Mac from the site – the launcher says Unreal Engine 4.5.1 not installed - why is not everything included in the Mac download? I’m using a 2013 iMac running Yosemite- otherwise vanilla.

The .dmg only installed the Launcher. You use the Launcher to download and update the engine. In the Launcher, open the Library tab. At the top, click the +Add Engine button. When the new slot appears, select the version of the engine you would like to install (latest is 4.5.1) and click the Install button. This will download that version of the engine to the same drive you installed the Launcher on. Once that has finished, you will have UE4 on your system. Hope that helps!
