I have just had this exact problem in my renderings, when on-screen everything looked great.
I went through a number of settings, but found the problem is with spacial and temporal samples together. I set spacial samples to 1 and then temporal samples to 256. Everything rendered out as expected.
It might be worth noting I also checked the “Render Warm Up Frames” and changed the Engine warm up count to 120.
