It seems to be a common issue but none of the solutions that I have found on the forums have worked yet.
I have tried all of the advices here to no avail.
Help would be appreciated and since I have tried all the existing solutions there may be another that could be added to the troubleshooting page.

In your Dxdiag it lists your Graphics card as Intel HD Graphics 4000, do you also have a onboard card as well? If so, post which one and make sure it’s driver is up to date.
Also, the Launcher Logs you posted look to be the standard logs. Could you follow the instructions here to generate your Debug Logs?
NVIDIA GeForce 720M - Drivers are up to date on this one.
