The same issue has happened to our team a few times. We had thought the issue cleared up since updating software but unfortunately we still get it occasionally. We keep our inputs in a folder separate from the project where it is very clear not to move, delete, or edit the content so it is 100% not a matter of manipulating the inputs etc.
