It seems that the issue may be due to your Nvidia Quadro 5000 card. Could you provide me with the model number for that card or at least confirm that it is a 5000? I want to be sure that the DXDiag isn’t just listing the series it’s in.
Once I have this information, I’ll be able to test a similar card (Most likely a Quadro 4800 FX) to see if I can reproduce the same problem.
