Need Some Information Regarding The Notification of UE4 Project Release Form

Hello, I have two questions regarding the product notification/release form.

  1. For the ‘DEVELOPER NAME’ section of the form, do I need to use my legal full name if I am a Sole Proprietor ? Can I use my DBA name under which the product will be released on Steam and marketed to the public and use my legal full name only in the ‘DEVELOPER CONTACT PERSON’ section ? How private are these information? Will it ever be shared with the public ?

And

  1. Do I need to submit the form for releasing free games ? Also what about free uncompiled projects released for learning purposes or as part of a tutorial series ?
