Collision mesh gets Misaligned with render mesh. How, why?

Hi,

I have a character who when landing some fast strong hit’s on punch movements get its collider ‘mesh’ shape dis-located from the rendering mesh that it belongs to.
This is a convex hull type collider that is coupled with the mesh, set up inside the mesh itself.

How is this possible?
How can I prevent it? Does anyone know?
